Create Level Set
I know I've done this before but I can't find it. After extensive searching of the mostly useless Help file I did find this line: "Level Sets can be created, renamed or deleted through the context menus." however I cannot find the context menu to Create a level set. Perhaps someone can enlighten me?
Additionally, I have a file that has a reference in it and for some reason it will not display the reference in the Layout if I render it as anything other that Wireframe or Default, is there something I am missing?

So I did manage to find the "Create Level Set" option, it turns out you have to click directly on the "Level Sets" column header. I had assumed that clicking anywhere in the Level Sets part splitter to get the context menu would provide that option.
Now if someone could tell me how to get the reference to show up (and render) in my instance in layout mode I'll be cooking with gas.

Woohoo, I found the issue for this too. Apparently if you have "Solids only Hidden Line Rendering" set for the Instance it will not display the Reference, presumably because it isn't a "solid"?
