This is not directly related to your issue but I often find similar anomalies when using fasteners in my layout instances as well. I often have to insert fasteners into layouts as solids instead of fasteners. I do this one of two ways; On the Tools>Fastener window click the Fast Setup tab and un-check "Create as Fastener object" before placing the fastener or by bursting (Edit>Entities>Burst) the fastener after placing. Not sure if this will help in your case but anything is possible.
