Those mouse button assignments should stick right away. What if you try saving your workspace immediately after making those changes, do the button assignments stick?

As for the icons, the red X means that hte command is not able to find the image that is the iicon. So it sounds like the settings is not transfer over correctly. Either the paths are wrong or the bmp that make up the icons are missing.
